Alpha Healthcare Institute offers a wide range of health solution for families who need retirement living, assisted living, rehab, skilled nursing, and more. We are seeking an experience Licensed Practical Nurse (LPN), Full-Time / Part-Time to join our team. The primary role of potential LPN candidate is to assist in the coordinating care for our residents, which includes care planning, communicating with family and other care professionals, and executing triage assessments in accordance with current federal, state, and local standards and guidelines, the regulations that govern Alpha Healthcare Institute.
Responsibilities
- Ensure that appropriate care is being given in line with company standard professional practices and policies.
- Timely respond to client conditions and plan care accordingly.
- Develop, update, and manage resident care plans.
- Follow-up with resident concern notes and resolve concerns appropriately.
- Create accurate and complete documentation of observations and care.
- Obtain specimens for lab tests as ordered by Physician
- Manage the day-to-day activities performed by our Resident Aides.
- Work responsibly as a team member as well as an individual.
- Communicate effectively with residents, families, staff, vendors, doctors, and the general public.
- Maintain confidentiality of all client care information inside and outside of the company.
- Monitor care to ensure that all clients are treated fairly, and with kindness, dignity, and respect.
- Respond appropriately to phone calls concerning resident care and staff inquiries 24/7 during on call weeks.
- Provide leadership, direction, and education to Resident Aide’s in their daily activities.
Qualifications
- Must be a Licensed Practical Nurse (LPN) in the State of Illinois and be in good standing.
- Associate Degrees, Nursing (Preferred)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Required)
- Other releated certification
- Ability to pass a physical, drug screen, 50-pound lift test, fingerprinting, and a background check.
- Previous experience in long-term care is plus
